Smoking is the leading cause of preventable disease, disability, and death in the U.S.
No matter how it's delivered, nicotine is harmful for youth and young adults. E-cigarettes typically contain nicotine as well as other chemicals that are known to damage health.
Quitting is hard, but there is help available. The more resources you use to quit, the better your chances of staying tobacco-free.

Medicaid Managed Long Term Care (MLTC) plans are insurance companies that provide services to chronically ill and disabled Medicaid beneficiaries who need long term care services and support. MLTC plans have provider networks including home care agencies, adult day programs, and other long term care providers. Plan members must choose from providers within a plan’s network. Plans are approved and monitored by the NYS Department of Health. Consumer directed services are available.
Eligibility:
• Must have a chronic illness or disability that makes you eligible for services usually provided in a nursing home
• Are able to stay safely at home at the time you join the plan
• Are expected to need long-term care services for at least 120 days from the date you enroll
• Must have approved Medicaid
• Meet the age requirement of the plan
• Live in the area served by the plan

Medicaid Managed Long Term Care (MLTC)
Supports and Services for People with Developmental Disabilities
People with developmental disabilities can apply for OPWDD supports and services which allow them to remain in the community. Services can include habilitation services, respite care, service coordination, and adaptive technologies. Individuals, or their caregivers, guardians, advocates, or care coordinators who help them, may apply. While some services are funded through the county and/or state government, most are funded by the federal government through Medicaid. The New York State Office for People with Developmental Disabilities (OPWDD) now has the Front Door initiative. The new Front Door approach gives individuals who are seeking services for the first time or who are interested in modifying their current plans as many opportunities as possible to direct their own services.
Eligibility:
• Diagnosis of a developmental disability
• The disability has occurred before the person was 22 years of age
• The disability can be expected to continue indefinitely, or permanently
• The disability causes a substantial handicap to a person’s ability to function normally in society

Traumatic Brain Injury (TBI) HCBS Medicaid Waiver
The Traumatic Brain Injury Waiver is one component of a comprehensive strategy developed by the NYS Department of Health to prevent unnecessary entrances into nursing homes and to help individuals leave nursing homes to live in a community based setting. The waiver provides Medicaid-funded services needed for participants to live in the community. Participants may be eligible for rent subsidies and housing supports.
• Diagnosis of Traumatic Brain Injury, or related diagnosis
• Adults age 18-64 years of age
• Be a Medicaid recipient
• Eligible for nursing home level of care
• Choose to live in the community with assistance from available supports and programs

How can the Health Home CM help me?
The Health Home (HH) Care Manager (CM) assists you to:
Coordinate your care through effective communication with other providers
Advocate on your behalf so you receive the care you deserve
Navigate the complex healthcare system
Link to resources for finding affordable housing, food, clothing, childcare and transportation
Learn more about meaningful activities to improve your health and keep you healthy
Identify your goals and ensure everyone involved in your care understands your goals and the care plan you created, to best help you reach those goals
Who is Eligible?
Medicaid recipients, including those who are already members of a Managed Care Organization. Individuals who have both Medicaid and Medicare are also eligible.
Those eligible need to meet one of the following conditions:
Two or more chronic health conditions, such as asthma, diabetes, heart disease, mental health condition or substance use disorder, (list of chronic conditions) or
A significant Mental Illness, or
Living with HIV/AIDs
