The Mental Health Community Partners Program (MHCP) supports the recovery of adults and youth aged 12+ with HIGH mental health needs in transitioning from unwell to well (for example, being discharged from a mental health facility and returning to their family and community).
MHCP uses individuals trained and certified in the program to provide this support. MHANYS believes family engagement during this time of transition to be critically important in helping adults and youth achieve and maintain long-term wellness. The MHCP program provides this support for 90 days to the adults and youth in recovery, as well as their family members and other chosen supporters.
What to expect from your coach:
Meet with the participant and their supporters weekly throughout the 90-day program for up to 1 hour.
Guide participants in learning a psychoeducational curriculum focused on the cycle of recovery and whole-family support.
Provide ongoing support to prevent re-hospitalization and support the transition to the community.
